Membership Information
You must be at least 16 years old and have an interest in providing emergency medical service to the community or assisting (non-riding member) those who do. For information contact info@ccvac.us with your name, address and phone number or call 914-739-0881 leaving your name and phone number and someone will contact you shortly.
Our Application can now be filled out on line and emailed to our membership committee chairperson. You should reside within close proximity to the corps since we respond from home and do not have crews assigned in quarters.

Instructions
1. Fill out the application and copy it to an email message and sent it to membership@ccvac.us, or print and fax it to 914-739-8573, or print it and deliver it to 6 Kings Ferry Road Montrose, NY 10548 or mail it to PO Box 275 Montrose NY 10548. Make sure all questions are answered.
2. When we have the application we will contact you to set up an interview.
